TÜRKİYE'DE PANDEMİ HASTANESİ OLMAYAN BİR SAĞLIK KURULUŞUNDAKİ MEME GÖRÜNTÜLEME ÜNİTESİNİN ÇALIŞMALARINA COVID-19 PANDEMİSİNİN ETKİSİ

Öz

AMAÇ: COVID-19 salgınının, Türkiye'de pandemi hastanesi olmayan bir sağlık kuruluşunun meme görüntüleme ünitesindeki hasta başvuru sayıları, hastalara yapılan işlemler ve hasta görüntüleme bulguları üzerindeki etkisini değerlendirmeyi amaçladık.
GEREÇ VE YÖNTEM: Çalışmaya, pandemi öncesi ve pandemi dönemi sırasında ünitemize başvuran sırasıyla 2010 ve 740 hasta dahil edildi ve retrospektif olarak değerlendirildi. Hastaların cinsiyeti, yaşı, neoadjuvan kemoterapi hikayesi, hastalara uygulanan görüntüleme yöntemleri, tetkikler sonrası elde edilen BI-RADS sonucu ve uygulanmışsa biyopsi ve/veya tel lokalizasyon işlemleri not edilip dönemler arasında karşılaştırıldı.
BULGULAR: Pandemi döneminde, ünitemize başvuran hasta sayısında ve yapılan mamografi, ultrason, MRG, biyopsi ve tel lokalizasyon işlemlerinin sayısında istatistiksel anlamlı azalma görüldü (p <0,001). Tarama mamografi oranı da pandemi döneminde istatistiksel anlamlı olarak azaldı (p <0,001). Pandemi döneminde elde edilen tetkik sonuçlarında BI-RADS 1 oranı, pandemi öncesi döneme göre istatistiksel anlamlı olarak azaldı (p = 0,044).
SONUÇ: Meme görüntüleme ünitemize, başta mamografi taraması olmak üzere tüm görüntüleme yöntemleri, biyopsi ve tel lokalizasyonu için başvuran hasta sayısı pandemi döneminde önemli ölçüde azalmıştır. Bu işlemlerdeki sayısal azalma gelecekte meme karsinomu teşhisinde artışa neden olabilir. Ancak hastanemizin pandemi hastanesi olmaması, neoadjuvan kemoterapi alan hastalar için güvenli bir ortam yaratmıştır.

IMPACT OF THE COVID-19 PANDEMIC ON THE OPERATIONS OF THE BREAST IMAGING UNIT OF A NON-PANDEMIC HOSPITAL IN TURKEY

Öz

AIM: We aimed to evaluate changes in patient numbers, procedures performed, and the result of imaging procedures in the breast imaging unit of a non-pandemic hospital in Turkey during the COVID-19 pandemic.
MATERIAL AND METHOD: The study retrospectively evaluated 2010 and 740 patients who presented to our unit in the pandemic and non-pandemic periods, respectively. Patient sex and age, neoadjuvant chemotherapy story, imaging modalities used, final BI-RADS classification, and biopsy and/or wire localization procedures were compared between the periods.
RESULTS: There were significant decreases in the number of patients who presented to our unit and the numbers of mammography, ultrasound, MRI, biopsy, and wire localization procedures performed during the pandemic period (p<0.001). The screening mammography rate also decreased significantly during the pandemic period (p<0.001), and there was a statistically significant decrease in the rate of BI-RADS 1 diagnoses during the pandemic period compared to the non-pandemic period (p=0.044).
CONCLUSION: The number of patients presenting to our breast imaging unit for biopsy, wire localization, and all imaging modalities, especially screening mammography, decreased significantly during the pandemic period. Decline in the procedures may lead an increase in breast carcinoma diagnosis in the future. However, the fact that our hospital is a non-pandemic hospital seems to have created a safe environment for patients receiving neoadjuvant chemotherapy.
