Preterm Bebek Annelerinin Maternal Bağlanma, Emzirme Özyeterliliği ve Bakıma Katılım Düzeyleri

Abstract

ÖZ
Bu çalışma preterm bebeği olan annelerin maternal bağlanma, emzirme öz-yeterliliği ve bakıma katılım düzeylerinin ve aralarındaki ilişkisinin belirlenmesi amacıyla ilişkisel-tanımlayıcı ve kesitsel tasarımda gerçekleştirilmiştir. Çalışmanın örneklemini 289 preterm bebek annesi oluşturmuştur. Veriler Kişisel Bilgi Formu, Maternal Bağlanma Ölçeği (MBÖ), Emzirme Öz-Yeterlilik Ölçeği (EÖYÖ) ve Bakıma Katılımı Değerlendirme Ölçeği (BKDÖ) kullanılarak toplanmış, istatistiksel analizinde tanımlayıcı testler, Kruskall-Wallis, Mann-Whitney U ve Benferroni testi kullanılmıştır. Araştırmaya katılan annelerin %32.5’inin 23-27 yaş aralığında, %34.9’unun ilkokul mezunu olduğu, %70.9’unun çalışmadığı, %86,2’sinin sağlık güvencesi olduğu, ve 46.7’sinin gelirini giderine denk olduğu belirlenmiştir. Preterm bebeklerin %52.9’unun kız, %63’ünün 33-38 haftada doğduğu, ortalama doğum ağırlığının 1973±654,82 gr ve %81.7’sinin 2500 gr altında doğduğu, bebeklerin %73.4’ünün YYBÜ’de yattığı belirlenmiştir. Bu çalışmada; preterm bebek annelerinin maternal bağlanma düzeyinin düşük (31.26±6.14), emzirme öz-yeterliliğinin iyi (56.01±10.74) ve bebeğin bakımına katılımının çok iyi düzeyde (16.19±2.87) olduğu belirlenmiştir. Annelerin maternal bağlanma puanları ile emzirme öz-yeterlilik ve bakıma katılımı değerlendirme puanları arasında anlamlı ilişki olduğu; annenin bakıma katılım düzeyi arttıkça maternal bağlanma düzeyinin de arttığı, maternal bağlanma düzeyi arttıkça emzirme öz-yeterliliğinin de arttığı saptanmıştır. Yeni doğan ünitelerinde ebe ve hemşireler anne-bebek bağlılığını ve annenin emzirme öz-yeterliğini etkileyen etkenleri belirlemeli, anne-bebek iletişimini sağlamaya yönelik eğitim ve danışmanlık yapmalıdır.

MATERNAL ATTACHMENT, BREASTFEEDING SELF-EFFICIENCY AND CARE PARTICIPATION LEVELS OF MOTHERS OF PRETERM BABIES

Abstract

ABSTRACT
This study was conducted in a relational-descriptive and cross-sectional design to determine the levels of maternal attachment, breastfeeding self-efficacy and participation in care of mothers with preterm babies and the relationship between them. The sample of the study consisted of 289 mothers of preterm babies. Data were collected using the Personal Information Form, Maternal Attachment Scale (MBÖ), Breastfeeding Self-Efficacy Scale (EÖS) and Care Participation Evaluation Scale (BKDS). Descriptive tests, Kruskall-Wallis, Mann-Whitney U and Benferroni tests were used in statistical analysis. It was determined that 32.5% of the mothers participating in the research were between the ages of 23-27, 34.9% were primary school graduates, 70.9% were not working, 86.2% had health insurance and 46.7% of them had income equal to their expenses. It was determined that 52.9% of preterm babies were girls, 63% were born at 33-38 weeks, the average birth weight was 1973±654.82 g and 81.7% were born below 2500 g, and 73.4% of the babies were hospitalized in the NICU. In this study; It was determined that the maternal attachment level of mothers of preterm babies was low (31.26±6.14), their breastfeeding self-efficacy was good (56.01±10.74) and their participation in the baby's care was very good (16.19±2.87). There is a significant relationship between mothers' maternal attachment scores and breastfeeding self-efficacy and care participation evaluation scores; It was found that as the mother's level of participation in care increased, the level of maternal attachment also increased, and as the level of maternal attachment increased, breastfeeding self-efficacy also increased. Midwives and nurses in neonatal units should determine the factors affecting mother-baby attachment and the mother's breastfeeding self-efficacy, and provide training and consultancy to ensure mother-baby communication.
