Total Abdominal Histerektomi + Bileteral Salfingooferektomi (TAH-BSO) Ameliyatı Olan Hastanın Gordon’un Fonksiyonel Sağlık Örüntülerine Göre Hemşirelik Bakımı: Olgu Sunumu

Abstract

Menopoz, kadınların fiziksel ve psikolojik değişimler yaşadığı önemli bir yaşam evresidir. Menopoz doğal süreçte kendiliğinden ya da cerrahi olarak ortaya çıkabilir. Bu süreçte hemşirelerin kadınların tedavi, bakım ve izleminde önemli rolleri vardır. Bakım ve tedavinin etkinliğini artırmak için model ve kuramların kullanımı oldukça önemlidir. Bu çalışmada bir devlet hastanesine başvuran ve TAH+BSO ameliyatı geçiren bir hasta ameliyat sonrası dönemde dört gün boyunca takip edilmiştir. Hasta için Gordon Fonksiyonel Sağlık Örüntüleri Modeli kapsamında bir bakım süreci oluşturulmuş ve 11 fonksiyonel bakım ihtiyacı belirlenmiştir. Bu örüntüler; sağlık algısı-sağlık yönetimi örüntüsü, beslenme-metabolik örüntü, eliminasyon örüntüsü, aktivite-egzersiz örüntüsü, uyku-dinlenme örüntüsü, bilişsel-algısal örüntü, benlik algısı-benlik kavramı örüntüsü, rol-ilişki örüntüsü, cinsellik-üreme örüntüsü, baş etme-stres toleransı örüntüsü ve inanç-değer örüntüsüdür. Örüntüler kapsamında ağrı, düşme riski, bulantı ve kusma, aktivite intoleransı, yorgunluk, rol ve ilişkilerde bozulma, ölüm kaygısı, stresle etkisiz baş etme, cinsel örüntüde bozulma hemşirelik tanıları konuldu. Olgu sunumu için hastadan sözlü ve yazılı onam alınmıştır. Bu çalışma ile Gordon fonksiyonel sağlık örüntüleri modelinin hastaya verilen bakımda yararlı olduğu sonucuna ulaşılmıştır. Bu anlamda modelin jinekoloji ve onkoloji alanlarında hasta bakımında kullanılması verilen hemşirelik bakımının etkinliğini artırmada faydalı olacaktır.

Abstract

Menopause is an important life stage in which women experience physical and psychological changes. Menopause can occur spontaneously in the natural process or surgically. In this process, nurses have important roles in the treatment, care and follow-up of women. The use of models and theories is very important to increase the effectiveness of care and treatment. In this study, a patient who applied to a state hospital and underwent TAH+BSO surgery was followed up for four days in the postoperative period. A care process was created for the patient within the scope of the Gordon Functional Health Patterns Model, and 11 functional care needs were determined. These patterns are health perception-health management pattern, nutritional-metabolic pattern, elimination pattern, activity-exercise pattern, sleep-rest pattern, cognitiveperceptual pattern, self-perception-self-concept pattern, role-relationship pattern, sexuality-reproductive pattern, coping-stress tolerance pattern, and belief- value pattern.Within the scope of patterns, nursing diagnoses of pain, fall risk, nausea and vomiting, activity intolerance, fatigue, deterioration in roles and relationships, death anxiety, ineffective coping with stress, deterioration in sexual pattern were made. Verbal and written consent was obtained from the patient for the case report. With this study, it was concluded that Gordon functional health patterns model was useful in the care given to the patient. In this sense, the use of the model in patient care in the fields of gynaecology and oncology will be beneficial in increasing the effectiveness of the nursing care provided.
